Employment Application Signature and Notices

Pre-Employment Reference Check Authorization

  • I have applied for employment with Lunchtime Solutions Inc. and have provided information about my education, previous employment and references. My signature below authorizes my former employers, educational institutions and references to release the contents of my records and to provide any additional information that may be necessary for my application for employment to Lunchtime Solutions Inc., whether the information is positive or negative. I hereby authorize Lunchtime Solutions Inc. to investigate all statements made in my application for employment and to obtain any and all information concerning my former employment, education and references. This includes my academic transcripts, job performance appraisals/evaluations, wage history, disciplinary action(s) if any, and all other matters pertaining to my employment and educational history. I knowingly and voluntarily release all former employers, educators, references and Lunchtime Solutions Inc. from any liability arising from their giving or receiving information about my employment history, my academic credentials or qualifications and my suitability for employment with Lunchtime Solutions Inc. This form may be photocopied or reproduced as facsimile, and these copies will be as effective as a release or consent as the original which I sign.

Authorization for Criminal Records and Previous Employment Check

  • I, the undersigned, authorize Lunchtime Solutions Inc., and any agency that may be utilized by Lunchtime Solutions Inc, to conduct a criminal history file check, investigation by name and identifiers, consumer credit check, or driving record check as necessary for consideration of my application. Background checks will include verification of information provided on the completed application for employment, the applicant’s resume or on other forms used in the hiring process. Information to be verified includes, but is not limited to, social security number and previous addresses. Lunchtime Solutions, Inc. may also conduct a reference check and verification of the applicant’s education and employment background as stated on the employment application or other documents listed above. The background check may also include a criminal record check. If a conviction is discovered, a determination will be made whether the conviction is related to the position for which the individual is applying or would present safety or security risks before an employment decision is made. Additional checks such as a driving record or credit report may be made on applicants for particular job categories if appropriate and job related. If an applicant is denied employment wholly or partly because of information obtained in an employment check conducted by the company’s consumer credit reporting agency, the applicant will be informed of this and given the name, address and phone number of the vendor to contact if he or she has specific questions about the result of the check or wants to dispute its accuracy. Any applicant who provides misleading, erroneous or willfully deceptive information to Lunchtime Solutions, Inc. on an employment form or resume or in a selection interview is immediately eliminated from further consideration for employment with Lunchtime Solutions, Inc.
